Forms for Students

These forms for graduate students can either be filled out online or in Adobe Reader or Acrobat after downloading and saving them to your computer.

 

  • M.A. Students
  • Advancement to Candidacy for M.A. form

    Save this form to disk and fill it out in Adobe Reader or Acrobat or print it and fill it out by hand. Send or give the filled-out form to your graduate advisor for signature. It should then go to the graduate coordinator (Cat Huff), who will file it with the Office of Graduate Studies. The completed form should be filed not before at least one-half of all course requirements have been completed, and no later than the quarter preceding expected completion of all requirements for the degree, with attention paid to the filing deadlines.


    Committee members for M.A. (comprehensive) or Ph.D. (preliminary) exam

    The graduate advisor, in consultation with the student, nominates a chair and two other faculty members to serve on the M.A. Comprehensive Exam Committee or the Ph.D. Preliminary Examination Committee. After consulting with the graduate advisor, the student should contact each prospective committee member, discuss the project, and on this form obtain signatures of individuals who agree to serve as committee members. Please return the form to the Graduate Coordinator for placement in the student academic file.


    Report on Comprehensive Exam for M.A.

    This form should be used to report the outcome of the committee review of a comprehensive exam paper for the M.A. degree. A passing score is a prerequisite for completion of the degree requirements for M.A. students.


    Report of Completion of M.A.

    This form is to be submitted after completion of all coursework requirements for the M.A. degree and after successful completion of the comprehensive exam paper (or preliminary exam paper/QP 1 for Ph.D. students earning an M.A. en route). Students should fill out applicable portions, print, obtain signatures from the comp/prelim paper committee chair and graduate advisor, and submit the completed form to the graduate coordinator.

  • Ph.D. Students

  • Qualifying Examination Application for Ph.D. Degree

    Save this form to disk and fill it out in Adobe Reader or Acrobat, or print it and fill it out by hand. Ask your graduate advisor to sign the completed form and give it to the graduate coordinator for submission and filing. This form should be completed after the qualifying exam committee has been selected, in consultation with your major professor, and filed with the Office of Graduate Studies at least four weeks prior to the scheduled date for the exam.


    Major Professor Selection Form

    Use this form to indicate your major professor in the Ph.D. program. You may save it to disk, open it in Adobe Reader (or Acrobat), version 8 or later, fill it out and sign it electronically (or not) and send it electronically (or not) to the graduate coordinator for further processing.


    Report on Preliminary Exam for the Ph.D.

    Use this form to indicate the results of a preliminary exam paper review for the Ph.D. program. You may save it to disk, open it in Adobe Reader (or Acrobat), version 8 or later, fill it out and sign it electronically (or not) and send it electronically (or not) to the graduate coordinator for further processing.


    Advancement to Candidacy for Ph.D. form

    Download and save to disk. Fill out applicable portions in Adobe Reader or Acrobat. Print, obtain all required signatures, and give it to the graduate coordinator to file with the Office of Graduate Studies.

  • All Graduate Students

  • Variable-unit courses: To obtain permission to enroll in a variable-unit course (e.g., LIN 199, LIN 197T, LIN 299 or LIN 396), request a course registration number (CRN) from the authorizing instructor, who can obtain one via the "Classes I Teach" module that can be accessed when logged in to the password-protected MyUCDavis portal.
